The New York Questionnaire form, also known as the Vendor Information Exchange System (VENDEX), serves as a crucial tool for vendors wishing to engage in business with New York City. This form is designed to gather essential information from potential vendors, ensuring that the city adheres to its charter mandate of partnering only with responsible entities. Effective September 25, 2014, the questionnaires became fillable, allowing for easier completion; however, vendors must still print and submit paper copies. The form includes two main components: the Vendor Questionnaire and the Principal Questionnaire. Each section requires thorough responses, with a strict policy against leaving questions unanswered or marking them as "Not Applicable." Specific formatting guidelines apply to various fields, such as dates and identification numbers, to maintain consistency and accuracy. Additionally, the form provides instructions on how to handle extra information if needed, as well as contact details for assistance. Vendors must be diligent in completing the form, as any incomplete submissions may lead to rejection. The importance of this form cannot be understated, as it plays a vital role in the vendor selection process for city contracts.

VENDOR QUESTIONNAIRE

The Vendor Information Exchange System (VENDEX) includes two questionnaires – the vendor questionnaire and the principal questionnaire. These have been developed to collect information from vendors who wish to do business with New York City, to ensure that New York City obeys the mandate in its charter to do business only with responsible vendors.

Questionnaires may be obtained in paper format from the VENDEX Unit (212-341-0933) or downloaded from the NYC website at http://www.nyc.gov/vendex.

Questionnaires must be completed in paper format. All questions must be answered. A response of “Not Applicable (N/A)”, or the equivalent, is not acceptable. Answers must be typewritten or printed in ink. If more space is needed to respond, photocopy the corresponding section’s page, check the box that additional information is attached, and attach the photocopied page to the questionnaire.

The publication “Vendor’s Guide to VENDEX” provides assistance and explanation for the questionnaires, including definitions of terms or phrases written in bold face throughout the questionnaires. If you have not obtained a copy of this publication, please download a copy from the New York City web site, or contact the VENDEX Unit at 212-341-0933. Detailed Guide for Filling Out New York Questionnaire

Completing the New York Questionnaire form is a crucial step for vendors looking to engage in business with New York City.   5. Enter the name of the person completing the questionnaire along with their title, telephone number, fax number, and email address.
  6. Provide the principal executive office address, including street, floor/suite number, city, state, and zip code.
  7. List the primary place of business in the NYC metropolitan area with the same details as above.
  8. Check if there have been other primary places of business in the NYC area within the past five years, and provide that information on page 7 if applicable.
  9. Indicate the ownership status of the primary place of business by checking the relevant boxes (Owned, Rented, etc.).
  10. List the addresses of the three largest sites anticipated for work related to the contract.
  11. Provide the website address of the submitting vendor.
  12. Select the range for annual gross revenue by checking the appropriate box.
  13. Indicate the business category by checking all that apply.
  14. Fill in the DUNS number if applicable and indicate if the vendor is listed on a national or regional stock exchange.
  15. Provide the date the submitting vendor began business in New York City.
  16. Answer all questions in the questionnaire carefully, ensuring that all fields are completed. If additional space is needed, photocopy the relevant page and attach it.
  17. Review the questionnaire for accuracy and completeness. Make sure there are no unauthorized changes or alterations.
  18. Print the completed questionnaire and any additional pages for submission.
